SummaryThe incumbent is assigned as Social Worker (SW) in Integrated Care (IC) who works at the interface between primary care (PC) and/or specialty medicine (SM) in an integrated care model. The position is assigned to the Mental Health Service and requires extensive work with Veterans diagnosed with medical conditions - mental illness - and substance use disorders (SUD).
