UAB Temp Services is looking for The Medical Caseworker - Transportation Coordinator.

The Medical Caseworker - Transportation Coordinator will assist with entering timely transportation
services within our EMR and Bambi system. Medical Caseworker Transportation Coordinator will be
responsible for entering up to 25-30 rides a day for patients going to and from medical appointments at
Cooper Green Mercy Health Authority and referred external services. Coordinator will also be responsible
for communicating with patients, confirming demographic and contact information to ensure accuracy of
transportation pick up and drops. While coordinating with contracted vendors dispatch team, Cooper
Green Mercy Health social services department, staff and providers. Coordinator will be responsible for
maintaining accuracy and compliance with the transportation program and policy. Communicating any
changes or barriers with our transportation program to Department of Social Services Manager.

KEY RESPONSIBILITIES
* Medical Caseworker Transportation Coordinator will be responsible for entering up to 25-30 rides
a day for patients going to and from medical appointments at Cooper Green Mercy Health
Authority and referred external services.
* Provide case management services across various departments, including primary care, specialty
clinics, behavioral health, and resource center; including providing guidance and facilitation of
referrals to external community resources as needed.
* While coordinating with contracted vendors dispatch team, Cooper Green Mercy Health social
services department, staff and providers.
* Coordinator will be responsible for maintaining accuracy and compliance with the transportation
program and policy.
* Aid in managing electronic resource board, completing non-clinical referrals, assisting patients
with requests such as obtaining immunization records upon request. Directing non-enrollee
patients to the enrollment department to assist with enrolling and providing additional navigation
for resources.
* Collaborate with licensed Social Workers within the clinics of Cooper Green Mercy Health Services
Authority to ensure patients' social services needs are addressed.
* Communicating any changes or barriers with our transportation program to Department of Social
Services Manager.
* Other duties as assigned.

Proposed Hours: M-F = 8hrs/day; 5 days a week (40 hours) 8-4:30pm, 8:30-5pm, 7:30-4pm (Depending on needs of department)

Medical Caseworker Transportation Coordinator

Required: Bachelor's degree in social work is required, with a minimum of one (1) to three (3) years of
experience in social services, case management or a similar role within a healthcare setting or related
field.
WORK ENVIRONMENT
* The on-site, office based role is full time, with the following options as work hours:
o 7:00 am - 3:30 pm CST
o 7:30 am - 4:00 pm CST
o 8:30 am - 5:00 pm CST
* This role may require lifting up to 50 lbs.
