The Discharge Planner is responsible for negotiating, procuring, and coordinating appropriate services and resources needed by patients after the acute inpatient phase. This role is capable of performing non-complex discharge planning tasks, such as returning patients to nursing homes, ordering durable medical equipment (DME), coordinating Home Health services, and faxing referrals to placements.
Responsibilities:
Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
Assists with appropriate interventions, working collaboratively with all healthcare team members regarding the need for discharge planning and available options.
Provides education as needed to enhance the healthcare team?s knowledge of case management services.
Coordinates discharge plans to ensure the complete and expedient movement of patients across the continuum of care.
Fosters positive professional relationships with contacts at various agencies and facilities of different levels of care.
Completes documentation related to consults, the status of pending placements, and discharge disposition of EMR.
Collaborates to identify discharge needs, including facilities, services, and DME equipment.
Participates in care planning preferences in assigned areas or assigned patient populations.
Provides support to social workers, counselors, and care managers.
Completes documentation for post-acute facilities, including chart copying as assigned.
Communicates confidently and effectively with all levels of hospital associates and physicians.
Possess excellent written and verbal skills.
Performs other duties as assigned.
Requirements:
Education/Skills
Bachelor's degree preferred.
Experience
A minimum of two years of experience in a medical setting is preferred.
