The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.
Position Summary
Reporting to the Senior Clinical Case Manager, the Clinical Case Manager provides high-level assessment, care planning, and coordination for students with chronic and acute complex medical conditions. This role ensures a collaborative patient-centered approach to care, facilitating communication and unity of effort amongst clinicians and integrating services across Columbia Health and external providers.
The Clinical Case Manager works as part of an interdepartmental team to ensure the delivery of high-quality, effective patient care. Meets the patient?s needs efficiently and expeditiously by continuously improving the patient?s experience and helping to ensure institutional standards of high-quality, effective patient care. The Clinical Case Manager, through broad knowledge of clinical care and systems management, helps optimize health outcomes by predicting, facilitating, and evaluating the patient?s movement through the continuum of care. The incumbent coordinates referrals to resources within the community to further address medical, mental health, behavioral health, and social needs of the patients.

Responsibilities
Care Coordination (35%)
Identify students with chronic and acute complex medical conditions and coordinate care in collaboration with the healthcare team.
Manage referrals to appropriate levels of care, including on-and off-campus providers.
Assist students in establishing a primary care provider relationship to promote more coordinated and effective health promotion, prevention and interventional care.
Serve as a patient advocate, ensuring timely access to appropriate services and a seamless handoff to ensure that appropriate care and services are delivered as needed.
Build strong patient relationships to encourage early communication of risk factors and reduce crisis situations.
Use appropriate screening criteria, knowledge, and clinical judgment, to assess patient?s current functional level and develop and monitor ongoing care plans.
Intake and Case Review (35%)
Review clinical case histories of incoming students with chronic medical conditions.
Initiate follow-up communications with entering students, who have identified a significant or chronic medical condition and gather information from relevant sources to determine appropriate level of care.
Refer students to appropriate internal and external services as needed. Collaborate care with members of Columbia Health to effect optimal patient.
Clinical Support and Collaboration (20%)
Perform a wide range of nursing clinical duties to support Columbia Health's mission, including:
Facilitating appointments with community providers
Initiating and managing Prior Authorizations for medications and procedures.
Administering and teaching medication use.
Providing clinical coverage as required.
Participate in regular clinical supervision, case consultations, case management, and team meetings.
Represents Columbia Health Medical Services in relevant committees and programs, such as new student orientations.
Quality Improvement and Professional Development (5%)
Collaborate with various stakeholders to identify performance improvement and quality control initiatives.
Maintain current knowledge of patient-care resources, nursing practice, and case management standards.
Critically analyze and apply new research and best practices to care management.
Performs other duties as assigned (5%).
Minimum Qualifications
Bachelor?s degree and/or its equivalent experience required.
Current New York State LCSW license.
Minimum of 3 years of direct clinical or client-facing experience in healthcare or social services setting; preferably in nursing case management, community health, medical social work, or related field.
Strong independent judgment, initiative, discretion, and problem-solving skills.
Demonstrated ability to work with a diverse population and communicate effectively with students, providers, and campus partners.
Strong interpersonal skills and ability to foster effective working relationships.
Proficiency in computer systems and electronic health records.
Thorough understanding of acuity levels for a specific patient population.
Preferred Qualifications
Experience in a hospital, ambulatory, higher education, or similar setting.
Certified Case Manager (CCM) credential.
