The Health Behavior and Society Department is currently seeking a motivated and experienced Case Manager to provide case management and general client services at SPARC, a harm-reduction focused drop-by center and mobile outreach program for women and nomen in Baltimore, MD. The Case Manager will provide general case management services for SPARC guests, including but not limited to relationship building; needs assessments; internal and external care coordination; direct resource linkage; and progress monitoring.SPARC provides all services at no cost and aims to provide supportive services that are low-barrier and stigma-free.
