Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at CCH. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

Job Summary
The CCHIP Senior Integrated Care Specialist (Senior Specialist) will conduct HIV Testing, outreach (HIV awareness/lost to care), recruitment, and linkage to care service providing enhanced care coordination services to persons living with/affected by HIV/AIDS, as directed. The Senior Specialist will work closely with newly diagnosed HIV clients to ensure linkage to care patient navigation and medication adherence at Cook County Health (CCH) Ambulatory & Community Health Network (ACHN) Clinics. The Senior Specialist will promote program enrollment and the use of antiretroviral's (ARV) for HIV treatment and prevention. The Senior Specialist will oversee retention/case finding activities, to locate newly diagnosed HRSA eligible patients. The Senior Specialist will work with a team of peer educators and clinical staff who will assist with community outreach, HIV testing and health education onsite at ACHN locations and throughout eligible metropolitan area. This is a grant-funded position. Grant 2025 CCH Ryan White Title IV-PT D until July 31, 2026, with the potential of being renewed.
General Administrative Responsibilities
Collective Bargaining
* Review applicable Collective Bargaining Agreements and consult with Labor Relations to generate management proposals
* Participate in collective bargaining negotiations, caucus discussions and working meetings
Discipline
* Document, recommend, and effectuate discipline at all levels
* Work closely with labor relations and/or labor counsel to effectuate and enforce applicable Collective Bargaining Agreements
* Initiate, authorize, and complete disciplinary action pursuant to CCH system rules, policies, procedures, and provisions of applicable collective bargaining agreements
Supervision
* Direct and effectuate CCH management policies and practices
* Access and proficiently navigate CCH records system to obtain and review information necessary to execute provisions of applicable collective bargaining agreements
Management
* Contribute to the management of CCH staff and CCH' systemic development and success
* Discuss and develop CCH system policies and procedures
General Administrative Responsibilities continued
* Consistently use independent judgment to identify operational staffing issues and needs and perform the following functions as necessary: hire, transfer, suspend, layoff, recall, promote, discharge, assign, direct, or discipline employees pursuant to applicable Collective Bargaining Agreements
* Work with Labor Relations to discern past practice when necessary
Typical Duties
- Provides direct oversight of CCHIP Integrated Care Specialists and staff, in absence of the Sexual Health Clinic Coordinator (SHCC).
- Works alongside leadership to ensure operational and administrative duties are maintained, i.e., service delivery workflows and engagement of care.
- Serves as point of contact for CORE primary care patients need confirmatory testing.
- Assists with one-to-one training of CCHIP staff across all sites working in Sexual Health Clinic at CORE.
- Drafts and submits operational and productivity reports, including weekly reports on patient intervention and progress.
- Assists with the planning, implementation and evaluation of health education initiatives, i.e. training sessions, revisions of policies and procedures, etc.
- Monitors, maintains, and restocks inventory of supplies, including point of care testing kits, i.e., INSTI & Determine, as needed.
- Conducts outreach, recruitment, HIV Testing, pre/posttest counseling, and risk reduction counseling on at risk populations in clinical settings at assigned ACHN locations, including prevention and treatment.
- Conducts case reporting for CORE HRSA eligible patients, partner services, and documentation of health outcomes.
- Conducts linkage to care, health education/literacy assessment, specialty care referrals and enrollment and eligibility verification activities on newly diagnosed persons and/or out care persons, which may include HIV, STI, PrEP, & DoxyPEP education.
- Conducts sessions with CORE HRSA eligible patients newly diagnosed with HIV to link them to early intervention service.
- Partners with team members to conduct outreach to clients lost to care and home visits.
- Participates in daily/weekly case conferences and planning with care team.
- Engages HRSA eligible patients newly diagnosed and/or lost to care patient in primary care.
- Identifies PrEP eligible clients and links them to appropriate services.
- Assists in the implementation of PrEP services and appointment scheduling in primary care for PrEP ensuring clients receive education about coverage programs.
- Provides individual- and relationship-based HIV and STI counseling.
- Documents services rendered primarily within CCH Electronic Medical Records System (EMR) and CAREWare.
- Provides education about Post-Exposure Prophylaxis (PEP), and other effective biomedical and prevention interventions. Refers eligible, interested clients to primary care medical services at CCH sites to further discuss PEP and other preventive care with an SSHARC medical provider.
- Participates in weekly case conferences and planning with care coordination team in person or by phone.
- Develops a service plan of action for meeting client's assessed needs, documents health education sessions, and completes accurate charts and electronic medical records in preparation for Ryan White Chiago Department of Public Health (CDPH) Audits.
- Makes referral and linkages to HIV medical care, assist client with gathering necessary Eligibility documentation for Ryan White services (including but not limited to Identification and/or birth record documents, residency documentation and income Verification).
- Documents attempts, client encounters, and services delivered into the electronic medical record as prescribed. Maintains records and logs related to this function as indicated. Documents EIS, Outreach and NMCM services in CAREWare for applicable client.
- Enters client encounters and services delivered into the CAREWare System and electronic medical record as prescribed under this enhanced care coordination model.
- Conducts retention and appointment reminders
- Makes appropriate referrals to prevention targeted testing program. Assists with HIV navigation Services.
- Adheres to all applicable State, local, and Federal law.
- Participates in community testing and outreach events as necessary.
- Participates in continuing education in HIV, STIs, and PrEP and special issues as it relates to HRSA eligible patients to remain current.
- Serves as a liaison between frontline staff and program leadership to ensure accurate reporting, rapid-cycle problem-solving, and continuous program improvement.
- Supports program planning and evaluation through data collection, analysis, and reporting across platforms such as CAREWare.
- Collaborates with CORE and ACHN leadership to expand outreach efforts, integrate innovative biomedical interventions (e.g., PrEP, DoxyPEP), and improve service delivery models for HRSA-eligible patients
- Provides subject matter expertise on HIV prevention and linkage to care, including the navigation of PrEP, EIS (Early Intervention Services), and medication assistance programs.
- Contributes to quality improvement initiatives, including the review of programmatic workflows and client feedback to improve care coordination and service delivery.

QualificationsMinimum Qualifications
- Bachelors degree from an accredited college or university (Must provide official transcript at the time of interview)
- Two (2) years of experience in health education, care coordination, HIV/STI prevention or social service program delivery is required
- Demonstrated cultural competence and/or experience with diverse populations, including: MSM, those recently released from incarceration, sex workers, individuals of extremely low income, substance users and/or those in recovery, and homeless individuals is required
- Must be able to work non-traditional work hours including nights or weekends, as requested is required
- Must obtain Chicago Department of Public Health (CDPH) or Illinois (IDPH) HIV Testing & Counseling certification within three (3) months of starting this employment is required
- Must be trained and certified in CDPH CHIMS system for case reporting and partner services within six (6) months of starting employment is required
- Proficiency using Microsoft Office (Word, Excel) is required
Preferred Qualifications
- Bachelor's degree or higher in a health-related field, education, psychology, social science from an accredited degree or higher from an accredited college or university is preferred (Must provide official transcript at the time of interview)
- Three (3) of HIV care and support services experience is preferred
- Experience working in a hospital, outpatient or community setting is preferred
- Proficiency using CAREWare is preferred
- HIV Testing and Counseling certification is preferred
- Disease Intervention Specialist (DIS) certification is preferred
- Bilingual is preferred
