The Community Health Worker (CHW) is a frontline public health worker who is a trusted team member with an in-depth understanding of the community served. The OSU Total Health Care Center (THCC) primary care clinic, a Federally Qualified Health Center, is adding a CHW to our team to serve as a link between health/social services and the community to better facilitate access to services and improve the quality and cultural competence of these services. The CHW builds individual and community capacity and awareness by increasing health knowledge and self-sufficiency through a range of activities, such as outreach, community education, intensive case management, social support, and advocacy. The CHW assists patients in
