Job Description Traveling to and working within both Union and Somerset Counties. Please note that travel to these counties is a required part of the position. 1. Performs comprehensive psychosocial assessments * Identifies behaviors and issues that affect coping at the end of life * Identifies issues that may have an impact on the safety of patients and families Identifies availability of support systems to meet patient/family goals * Identifies bereavement risks and communicates these appropriately 2.
