Responsible for supporting prior authorizations for inpatient psychiatric treatment, managing referrals, and coordinating care to ensure timely and appropriate access to inpatient services. Conducts assessments for patients with frequent or complex care needs and facilitates effective connection to community resources and ongoing support systems. Assists with diagnostic clarification, post-discharge treatment recommendation, identification of treatment options, high-risk behavior mitigation and prevention planning. Advises patient, family and responsible medical providers, as needed, to improve quality and continuum of care. Participates in hospital and community initiatives that address mental health and substance use issues. Provides expertise in mental health and substance use issues to the multi-disciplinary team including in-service education and support. Maintains a working knowledge of relevant behavioral health, medical, legal issues that impact patients and the hospital system. Documents assessment, dispositions, interactions conversations etc. according to department guidelines and standards. Can work independently, can prioritize and manage time efficiently. Flexibility in accepting diverse work assignments and managing stress related to change and the ability to resolve/ manage conflict.
